To request a new or modify an existing Cellular SID:

• Send the completed Request for New or Modified SID Form with $525 payment to:

                     NECA
                     SID Assignment

                     Whippany, NJ  07981

• The completed form may also be faxed to (973) 884-8082  Attn.: SID Assignment, or emailed to SID@neca.org.

• Payment may be made by credit card, check or money order.  Checks and money orders should be made payable to NECA.  Please enter the licensee name in the Memo section of the check.

SID assignments will be made within two weeks of our confirmation of payment and verification of information submitted.
